Primary dysmenorrhea results from increased prostaglandin production without underlying pathology, while secondary dysmenorrhea is caused by underlying conditions such as endometriosis or ovarian cysts. In adolescents, primary dysmenorrhea is most common and typically responds to nonsteroidal anti-inflammatory drugs (NSAIDs) and hormonal contraception.
