Leptomeningeal enhancement refers to abnormal enhancement of the leptomeninges (pia mater and arachnoid) on contrast-enhanced MRI or CT imaging. This finding suggests inflammation, infection, or malignant infiltration of these delicate membrane layers surrounding the brain and spinal cord. Common causes include meningitis, carcinomatous meningitis, sarcoidosis, and autoimmune conditions. The enhancement pattern can be focal or diffuse and may follow sulci and cisterns.

Usage note: Distinct from pachymeningeal (dural) enhancement; leptomeninges are the inner meningeal layers.
