Pseudobulbar palsy is a neurological condition resulting from bilateral upper motor neuron lesions affecting the corticobulbar tracts that control cranial nerve nuclei. Unlike true bulbar palsy, which involves lower motor neuron dysfunction, pseudobulbar palsy presents with spastic rather than flaccid weakness. Clinical features include dysarthria with a spastic quality, dysphagia, a hyperactive gag reflex, and pathological laughing and crying (pseudobulbar affect). The condition commonly occurs in patients with multiple strokes, amyotrophic lateral sclerosis, multiple sclerosis, or other conditions affecting bilateral corticobulbar pathways.
