rapid cycling

Bipolar disorder pattern characterized by four or more mood episodes (manic, hypomanic, or depressive) within a 12-month period.

Full Definition

Rapid cycling is a course specifier for bipolar disorder defined by the occurrence of four or more distinct mood episodes within a 12-month period. These episodes can be any combination of manic, hypomanic, mixed, or major depressive episodes, with each episode meeting full duration and severity criteria or being terminated by treatment or switching to an episode of opposite polarity. Rapid cycling occurs in approximately 10-20% of bipolar patients and is associated with greater illness severity, higher suicide risk, poorer treatment response, and increased functional impairment. Risk factors include female gender, earlier age of onset, thyroid dysfunction, antidepressant use, and substance abuse. Treatment typically requires mood stabilizers and careful monitoring, as rapid cycling can be challenging to manage and may require specialized treatment approaches.

Usage

Usage note: Must meet full criteria for distinct mood episodes - distinguish from mood instability or mixed features.
