thought process

The logical flow and organization of a patient's thinking as observed during clinical examination.

Full Definition

Thought process refers to the form and organization of thinking, including the logical progression and coherence of ideas as they are expressed verbally. This is distinct from thought content (what the patient thinks about). Normal thought process is goal-directed, logical, and coherent. Abnormal thought processes include tangentiality (going off-topic), circumstantiality (excessive detail), loose associations (illogical connections), and flight of ideas (rapid jumping between topics). Mental status examinations routinely assess and document thought process abnormalities. Editors must differentiate between thought process disorders and thought content disturbances in psychiatric reports.
