Myocardial deformation measurement representing shortening in the long axis of the heart. Early marker of systolic dysfunction.

Full Definition

Longitudinal strain measures myocardial deformation in the base-to-apex direction, representing the percentage change in myocardial fiber length during systole. Global longitudinal strain (GLS) is calculated as the average of all segments, with normal values typically more negative than -18% to -20%. This parameter is more sensitive than ejection fraction for detecting early systolic dysfunction and can identify subclinical myocardial disease in conditions like chemotherapy cardiotoxicity, infiltrative cardiomyopathies, and ischemic heart disease. Reduced (less negative) longitudinal strain often precedes changes in ejection fraction by months to years.

Usage note: Express as negative percentage (e.g., '-18%'); more negative values indicate better function.
