It typically includes sections for medical history, contraindication screening, pre- and post-treatment instructions, photographic consent, and the patient's signature. Editors reviewing these forms must ensure procedural names, risk disclosures, and regulatory-required statements are accurately and consistently rendered. The term should not be used interchangeably with 'medical history form' or 'patient intake form', which serve different functions.
