Aortic wall calcification refers to the pathological deposition of calcium salts within the layers of the aortic wall, most commonly associated with advanced atherosclerotic disease, aging, or metabolic disorders. This condition significantly impacts surgical planning and technique as heavily calcified aortic walls are friable, difficult to suture, and prone to embolization during manipulation. Calcification may be focal or circumferential and can extend through all layers of the aortic wall. In endovascular procedures, severe calcification may prevent adequate seal and increase the risk of endoleak. Preoperative imaging assessment of calcification burden is crucial for determining optimal treatment approaches and anticipating technical challenges.
