Blue-toe syndrome is a clinical condition characterized by the sudden onset of painful, cyanotic discoloration of one or more toes in the presence of palpable pedal pulses. This paradoxical finding of digital ischemia with intact major vessel flow is typically caused by distal embolization of cholesterol crystals, platelet aggregates, or small thrombi from proximal arterial sources. Common etiologies include atherosclerotic plaque ulceration, aneurysms, arterial manipulation during procedures, or anticoagulant therapy. The syndrome may present as isolated digital involvement or as part of a systemic atheroembolic syndrome affecting multiple organs. Diagnosis requires identification and treatment of the embolic source to prevent recurrent episodes.
