Cystic adventitial disease is an uncommon vascular pathology characterized by the development of mucin-containing cysts within the adventitial layer of arteries, leading to external compression of the vessel lumen. The popliteal artery is most frequently affected, though other peripheral arteries can be involved. The condition typically presents in middle-aged men with claudication symptoms. Imaging demonstrates a characteristic crescent-shaped or multiloculated cystic mass adjacent to the artery with variable degrees of luminal narrowing. Cross-sectional imaging is essential for diagnosis, showing the relationship between the cyst and vessel wall.
