Impaired function of the Eustachian tube that disrupts pressure equalisation in the middle ear, contributing to conductive hearing loss and tympanometric abnormalities.

Full Definition

The Eustachian tube connects the middle ear to the nasopharynx and normally opens to equalise middle ear pressure with atmospheric pressure. Eustachian tube dysfunction (ETD) occurs when this tube fails to open appropriately (obstructive ETD) or remains abnormally patent (patulous ETD), resulting in pressure imbalance, muffled hearing, and, in chronic cases, middle ear effusion. Tympanometry typically reveals a type C tympanogram (negative middle ear pressure) in obstructive ETD. Editors must distinguish ETD from OME: ETD is a functional diagnosis whereas OME refers to the specific presence of fluid. The term 'patulous Eustachian tube' describes the opposite failure—chronic opening—and must not be conflated with obstructive ETD.
