Full Definition

Biliopancreatic Diversion with Duodenal Switch is the most complex bariatric procedure, combining a sleeve gastrectomy with extensive small bowel bypass to create significant malabsorption. The procedure preserves the pylorus and first portion of the duodenum while rerouting bile and pancreatic enzymes to mix with food only in the final 75-100 cm of small intestine (common channel). BPD-DS produces the greatest weight loss but carries the highest risk of nutritional deficiencies and requires intensive long-term monitoring. It is typically reserved for patients with BMI >50 kg/m² or failed previous procedures.

Usage note: Use hyphen in BPD-DS. Distinguish from standard BPD (Biliopancreatic Diversion) which is rarely performed.
