Adenocarcinoma in situ of the cervix represents severe glandular dysplasia with features of adenocarcinoma but confined to the epithelium without invasion through the basement membrane. AIS is considered the immediate precursor to invasive cervical adenocarcinoma and requires definitive treatment, typically with cone biopsy or hysterectomy depending on patient factors. This lesion is strongly associated with high-risk HPV types, particularly HPV 18, and may be more difficult to detect with routine cytologic screening compared to squamous lesions. Complete excision with negative margins is essential due to the multifocal nature of the disease.
