Ventricular-arterial coupling describes the matching between ventricular contractile function and the arterial system's impedance to ejection. It is quantified as the ratio of arterial elastance (Ea) to ventricular elastance (Ees), with optimal mechanical efficiency occurring when Ea/Ees equals approximately 1.0. During cardiac anesthesia, this coupling can be significantly altered by anesthetic agents, surgical manipulation, and hemodynamic interventions. Understanding this relationship helps optimize inotropic and vasopressor therapy, particularly during weaning from cardiopulmonary bypass when both ventricular function and vascular tone may be compromised.
