Dormant conduction refers to the recovery of electrical continuity across tissue that was initially thought to be completely isolated following catheter ablation. This phenomenon typically becomes apparent when adenosine or other pharmacological agents are administered, revealing incomplete transmural lesions that maintain viable tissue bridges capable of conducting electrical impulses under certain conditions. Dormant conduction is particularly relevant in pulmonary vein isolation procedures, where its detection may predict arrhythmia recurrence. The identification of dormant conduction prompts additional ablation to achieve complete and durable isolation. This concept has led to routine adenosine testing in many laboratories to ensure procedural endpoint adequacy and improve long-term success rates.
