A cardiac magnetic resonance imaging technique that identifies areas of myocardial fibrosis and scar tissue through delayed enhancement with gadolinium-based contrast agents. Late gadolinium enhancement occurs because contrast agent washout is delayed in fibrotic tissue compared to normal myocardium. This imaging modality is crucial for ventricular tachycardia substrate assessment, providing detailed anatomic information about scar distribution, transmurality, and border zones that serve as potential arrhythmogenic substrates. LGE imaging guides ablation strategies and risk stratification in patients with cardiomyopathy. Integration with electroanatomic mapping systems allows correlation of electrical and anatomic abnormalities.
