Pulmonary vein isolation (PVI) is the cornerstone ablation strategy for atrial fibrillation, involving the creation of circumferential lesions around the pulmonary vein ostia to eliminate arrhythmogenic triggers. The procedure uses radiofrequency or cryoballoon energy to create transmural lesions that block electrical conduction between the pulmonary veins and left atrium. Success is confirmed by demonstrating entrance and exit block during the procedure. PVI has become the standard first-line invasive treatment for symptomatic atrial fibrillation refractory to antiarrhythmic medications.
