Slow pathway modification is a targeted ablation approach for treating atrioventricular nodal reentrant tachycardia by selectively eliminating or modifying the slow pathway component of dual AV nodal physiology. The procedure involves careful application of radiofrequency energy to the posteroseptal tricuspid annulus region where slow pathway inputs are located. Success is measured by elimination of dual AV nodal physiology or non-inducibility of AVNRT while maintaining normal AV conduction and avoiding heart block. This technique requires precise anatomical knowledge and careful titration of energy delivery to achieve therapeutic effect without damaging the compact AV node or His bundle.
