Stress perfusion MRI is an advanced cardiac imaging technique that evaluates myocardial blood flow and perfusion reserve using pharmacological stress agents (typically adenosine or regadenoson) combined with gadolinium-based contrast agents. The technique involves rapid imaging during first-pass perfusion of contrast material through the myocardium at rest and during peak pharmacological stress. Areas of reduced perfusion appear as dark regions on the images, indicating flow-limiting coronary stenosis or microvascular dysfunction. This non-invasive method offers high spatial resolution and excellent diagnostic accuracy for detecting coronary artery disease, with the additional advantage of comprehensive cardiac assessment including function, viability, and tissue characterization in a single examination.
