Hypothermic circulatory arrest (HCA) is an advanced perfusion technique used in complex cardiac and aortic surgery that involves cooling the patient to profound hypothermia (typically 15-20°C) and completely stopping circulation for a defined period. This technique provides optimal surgical exposure for procedures on the aortic arch, complex congenital repairs, or removal of cardiac tumors while protecting the brain and other vital organs from ischemic injury. The duration of safe circulatory arrest depends on the degree of hypothermia achieved, with typical safe periods ranging from 30-45 minutes. HCA requires specialized perfusion management and careful rewarming protocols.
