A complex neonatal cardiac surgery that represents the first of three staged procedures for hypoplastic left heart syndrome (HLHS). The operation reconstructs the aorta using the pulmonary artery and creates a new systemic circulation powered by the single right ventricle. Key components include aortic arch reconstruction, atrial septectomy, and creation of pulmonary blood flow via a Blalock-Taussig shunt or right ventricle-to-pulmonary artery conduit. This procedure is typically performed in the first week of life and is followed by the Glenn procedure and eventual Fontan completion.
