Complex lesion morphology describes coronary stenoses with anatomical features that make percutaneous intervention technically challenging and associated with higher complication rates. These characteristics may include severe calcification, chronic total occlusions, bifurcation involvement, extreme tortuosity, thrombus burden, ostial location, or long lesion length. The ACC/AHA lesion classification system categorizes lesions as Type A (simple), Type B1 (moderate complexity), Type B2 (moderate-high complexity), or Type C (high complexity) based on these morphological features. Recognition of complex morphology is crucial for procedure planning, device selection, and risk stratification.
