Cerebral desaturation refers to a clinically significant decrease in regional cerebral oxygen saturation (rSO2) as measured by near-infrared spectroscopy (NIRS). Typically defined as a reduction of 20% or more from baseline values or an absolute value below 50-55%, cerebral desaturation indicates potential cerebral hypoxia or ischemia. In cardiac surgery, this can result from hypotension, low cardiac output, hemodilution, embolic events, or malposition of cardiopulmonary bypass cannulas. Prompt recognition and correction of cerebral desaturation is crucial for preventing postoperative neurological complications, including stroke and cognitive dysfunction.
