Full Definition

Coronary perfusion pressure (CPP) represents the driving pressure for coronary blood flow and is calculated as the difference between aortic diastolic pressure and left ventricular end-diastolic pressure (or central venous pressure as an approximation). This parameter is critical in cardiothoracic anesthesia because it determines myocardial oxygen delivery, particularly to the subendocardium. A CPP below 40 mmHg is associated with myocardial ischemia, while optimal values typically exceed 60 mmHg. During cardiac surgery, maintaining adequate CPP is essential for myocardial protection, especially during weaning from cardiopulmonary bypass when the heart resumes ejecting against afterload.
