Pump lung syndrome, also known as post-perfusion lung syndrome, is a form of acute lung injury that develops after cardiopulmonary bypass. The condition results from systemic inflammatory response, complement activation, neutrophil sequestration in pulmonary capillaries, and increased capillary permeability. Clinical manifestations include impaired oxygenation, increased airway pressures, pulmonary edema, and prolonged mechanical ventilation requirements. Risk factors include prolonged bypass time, high transfusion requirements, and pre-existing pulmonary disease. Management focuses on lung-protective ventilation strategies and supportive care.
