Heart failure with preserved ejection fraction (HFpEF) is a clinical syndrome characterized by symptoms and signs of heart failure despite maintained left ventricular ejection fraction (typically ≥50%). Previously called diastolic heart failure, HFpEF involves impaired ventricular relaxation and increased filling pressures. It commonly affects elderly patients, particularly women, and is associated with hypertension, diabetes, and obesity. Diagnosis can be challenging as it requires demonstration of elevated filling pressures along with typical heart failure symptoms. Treatment focuses on managing comorbidities and symptom relief, as specific therapies proven to improve outcomes are limited.
