Full Definition

Dyskinesis refers to paradoxical outward motion of myocardial segments during systole, representing the most severe form of regional wall motion abnormality. Unlike akinesis where segments fail to contract, dyskinetic segments actually move away from the center of the ventricle during systole, often indicating transmural myocardial infarction with scar formation or ventricular aneurysm. Dyskinesis is typically seen in chronic infarct zones and is associated with poor functional recovery potential. On imaging, dyskinetic segments appear to bulge outward during systole and may be associated with wall thinning. The presence of dyskinesis often indicates need for surgical intervention such as aneurysmectomy.
