WMSI (Wall Motion Score Index) is a semiquantitative echocardiographic scoring system that assesses global left ventricular function by averaging wall motion scores across all 17 myocardial segments. Each segment is scored from 1 (normal) to 4 (dyskinetic), with the WMSI calculated as the sum of scores divided by the number of visualized segments. A WMSI of 1.0 indicates normal function, while values >1.7 suggest significant dysfunction. The index is particularly useful in stress echocardiography to quantify the extent of inducible ischemia and in clinical trials to standardize wall motion assessment. WMSI correlates with ejection fraction and provides prognostic information.
