Remnant cholesterol refers to the cholesterol carried in triglyceride-rich lipoproteins and their metabolic remnants, including VLDL, IDL, and chylomicron remnants. This cholesterol fraction can be calculated as total cholesterol minus LDL cholesterol minus HDL cholesterol, or measured directly. Elevated remnant cholesterol levels are independently associated with increased cardiovascular risk and may persist even when LDL cholesterol is well-controlled with statin therapy, representing a target for residual risk reduction.
