coronary perforation

A procedural complication involving breach of the coronary artery wall, classified by Ellis criteria based on severity.

Full Definition

Coronary perforation is a serious complication of percutaneous coronary intervention characterized by disruption of the arterial wall integrity. Perforations are classified using the Ellis classification: Type I (extraluminal crater without extravasation), Type II (pericardial or myocardial blush without contrast jet), Type III (frank streaming of contrast through a ≥1mm exit hole), and cavity spilling (perforation into anatomic cavity). Treatment depends on severity and may include prolonged balloon inflation, covered stent deployment, or emergency surgery.

Usage

Usage note: Always classify using Ellis criteria when documenting. Distinguish from dissection or extravasation.

In Context

  • "A Type II coronary perforation occurred during wire manipulation, managed with prolonged balloon inflation." — Complication management report
  • "No evidence of coronary perforation was observed throughout the procedure." — Safety documentation
