LIMA-to-LAD anastomosis represents the gold standard arterial conduit connection in coronary artery bypass grafting (CABG). The left internal mammary artery (LIMA) is harvested and connected end-to-side to the left anterior descending (LAD) coronary artery distal to significant stenosis. This anastomosis provides excellent long-term patency rates exceeding 90% at 10 years due to the arterial conduit's resistance to atherosclerosis. The procedure requires meticulous surgical technique to ensure optimal flow characteristics and avoid kinking. LIMA-to-LAD grafting significantly improves survival compared to venous grafts and is considered mandatory in most CABG procedures when technically feasible.
