Cortical tubers are focal developmental brain lesions characteristic of tuberous sclerosis complex, consisting of areas of cortical dysplasia with abnormal neuronal organization and increased cell size. These lesions appear as hyperintense areas on T2-weighted MRI and may show variable signal characteristics on different imaging sequences. Tubers are present from birth but may become more apparent with age due to delayed myelination in affected areas. While not all tubers are epileptogenic, they represent potential seizure sources and their location and characteristics help guide treatment decisions. The number and location of cortical tubers correlate with the likelihood of developing epilepsy and cognitive impairment.
