The fibular free flap is a microvascular reconstruction technique utilizing the fibula bone along with overlying skin and muscle for complex mandibular defect reconstruction. The flap is based on the peroneal artery and provides up to 25 centimeters of vascularized bone suitable for dental implant placement. The fibular free flap has become the gold standard for mandibular reconstruction due to its reliable blood supply, adequate bone stock for implants, and minimal donor site morbidity. The procedure requires microsurgical anastomosis to recipient vessels in the neck.
