A subciliary incision is a carefully placed surgical approach positioned 1-2mm below the lower eyelash line, running parallel to the lid margin. This incision provides excellent access to the orbital floor, infraorbital rim, and lower orbital wall while maintaining good cosmetic results. The technique is commonly used in orbital fracture repair, particularly for blowout fractures of the orbital floor. Proper execution requires careful attention to the anatomy of the lower eyelid to avoid complications such as ectropion, scleral show, or damage to the lacrimal system.
