Extrinsic Staining

Discolouration of the tooth surface caused by external chromogenic agents—such as tea, coffee, tobacco, or chlorhexidine—that deposit on the pellicle or within acquired deposits.

Full Definition

Extrinsic staining occurs on the outer surface of the tooth and is associated with dietary chromogens, tobacco products, metallic compounds (iron, copper), or medications such as chlorhexidine mouthwash. The stain is deposited within or on the dental pellicle and can generally be removed by professional polishing. In contrast, intrinsic staining originates within the tooth structure itself and cannot be removed by polishing. Dental hygienists must correctly classify staining before selecting an intervention, as polishing extrinsic stain is within scope of practice, whereas management of intrinsic stain may require restorative or bleaching procedures. Editors should ensure 'extrinsic' and 'intrinsic' staining are never conflated in clinical notes or educational materials.

Usage

Usage note: Contrast with 'intrinsic staining', which originates within the tooth and cannot be removed by polishing. Tetracycline staining and fluorosis are intrinsic, not extrinsic.
