Junctional Epithelium

The band of non-keratinized epithelial tissue that attaches the gingiva to the tooth surface at the base of the gingival sulcus. Its apical migration is a hallmark of periodontal attachment loss.

The junctional epithelium forms a cuff of epithelial cells that adheres to the tooth surface via hemidesmosomes and a basal lamina, sealing the periodontium from the oral environment. It is highly permeable and rapidly regenerates, but is also highly susceptible to bacterial challenge. When junctional epithelium migrates apically along the root surface, clinical attachment loss and pocket formation result. Editors must avoid confusing this term with 'sulcular epithelium,' which lines the lateral walls of the sulcus but does not attach to the tooth. Precise use of 'junctional' versus 'sulcular' is essential in histological and clinical descriptions.

Usage note: Do not use interchangeably with 'sulcular epithelium.' These are anatomically and functionally distinct structures.
