Crestal Approach Sinus Elevation

A minimally invasive technique for augmenting the maxillary sinus floor through the implant osteotomy site rather than a lateral window, using osteotomes or hydraulic pressure.

Full Definition

Crestal approach sinus elevation, also called the osteotome sinus floor elevation (OSFE) or transcrestal technique, accesses the maxillary sinus from the alveolar crest through the implant preparation site. Unlike the lateral window approach, it avoids a separate bony opening in the sinus wall, thereby reducing surgical morbidity. Bone graft material or simply hydraulic saline pressure is used to tent the Schneiderian membrane superiorly before or simultaneous with implant placement. The technique is best indicated when residual bone height is 5–8 mm. Editors should note the distinction between this term and 'sinus lift,' which is typically reserved for the lateral window approach.
