Several staging systems exist for osteoradionecrosis (ORN), with the Notani and Marx classifications being the most widely cited in the oral surgery literature. Staging typically progresses from early superficial mucosal exposure (Stage I) through deep bony involvement (Stages II and III), with Stage III indicating involvement of the inferior border of the mandible or fistula formation. Correct identification and consistent use of the specific staging system being referenced is an editorial obligation, as different systems use identical numerals to denote different severity levels. Authors should state the classification system by name at first use; editors should query any manuscript that uses stage numbers without this attribution.
