Charting symbols are the visual shorthand applied to dental charts—typically odontograms or periodontal charts—to document clinical findings such as crowns, missing teeth, caries, implants, and planned restorations. Editors working with dental records must recognise that incorrect or inconsistent symbol usage can constitute a documentation error with legal and billing implications.
