Clinical attachment loss (CAL) is a key diagnostic measurement in periodontal assessment, reflecting the cumulative destruction of the connective tissue and bone that support the tooth. It is calculated by combining the probing pocket depth with the distance from the gingival margin to the cementoenamel junction. Unlike pocket depth alone, attachment loss accounts for gingival recession and provides a more accurate picture of disease severity and progression. Editors working on clinical documentation, periodontal case narratives, or insurance submissions must not conflate 'attachment loss' with 'pocket depth'; the two measurements serve related but distinct diagnostic purposes, and using one in place of the other in narratives supporting periodontal codes can cause claim denials.
