Dental Claim Attachment

Supporting documentation—such as radiographic images, periodontal charts, photographs, or narratives—submitted alongside a dental insurance claim to substantiate the need for a procedure.

Full Definition

A dental claim attachment is any supplementary document or image submitted with or in response to a dental insurance claim to provide clinical evidence justifying the procedure billed. Common attachments include periapical and bitewing radiographs, periodontal charting records, clinical photographs, written dental narratives, and pre-operative study models. Attachments may be submitted proactively with the initial claim or reactively in response to a request for additional information (RAI) from the insurer. Editors reviewing billing guides, clinical documentation standards, or patient letters should be careful to distinguish 'claim attachment' from the narrative portion of the claim itself; a narrative is a written description within the claim, whereas an attachment is a separate supporting document.

Usage note: Distinguish from 'dental narrative,' which is a written description embedded within the claim form, and from 'predetermination,' which is a pre-treatment approval process that may also involve attachments.
