A perio chart is a standardised clinical document in which a dental hygienist or dentist records probing depths (typically six sites per tooth), bleeding on probing, furcation classifications, gingival margin positions, and mobility scores. Editors reviewing dental records or insurance submissions must distinguish this from a general odontogram, which maps restorations rather than periodontal status. In practice management software it is sometimes called a perio exam record. Accurate transcription of numeric probe readings is critical because errors directly affect diagnosis coding and insurance justification. The document is updated at every periodontal re-evaluation appointment.
