The improper practice of billing separate procedure codes for components of a single service that should be submitted under one inclusive code.

Unbundling in dental billing involves breaking down a procedure that CDT guidelines designate as a single, all-inclusive service into multiple component codes in order to increase reimbursement. A classic example is billing separately for a core build-up and a crown preparation when the build-up is considered inclusive to the crown procedure by certain payers. Editors proofreading claims or compliance reports must understand that unbundling is distinct from legitimate multiple-procedure billing. Insurance carriers use claim-scrubbing software to detect unbundling patterns, and detection can result in claim denial or fraud investigations. The concept is sometimes called 'fragmentation' in payer communications.
Usage
Usage note: Not to be confused with upcoding. Both involve improper billing, but unbundling splits one code into many, whereas upcoding substitutes a higher-value single code.
