Subgingival
Pronunciation: sub-JIN-jih-vul
Located below the gingival margin within the gingival sulcus or periodontal pocket; describes calculus, biofilm, restorative margins, or instrumentation occurring apical to the gum line.
Full Definition
Subgingival refers to the environment or structures found beneath the free gingival margin, within the sulcus or a pathological periodontal pocket. Subgingival calculus is dark brown or black due to incorporation of blood breakdown products, is more firmly adherent to root surfaces, and requires specialized scalers or ultrasonic instruments for removal. Subgingival microbiota are predominantly anaerobic and are closely associated with periodontal disease progression. Editors working on periodontal research manuscripts, clinical guidelines, or patient records should ensure consistent distinction between supragingival and subgingival contexts, as therapeutic implications differ markedly.
Usage
Usage note: Distinguish from 'infrabony', which describes the morphology of a periodontal bone defect rather than the location relative to the gingival margin.
