Post-cardiac arrest syndrome (PCAS) is a complex pathophysiologic condition that occurs after successful resuscitation from cardiac arrest and return of spontaneous circulation (ROSC). The syndrome encompasses multiple organ dysfunction including post-cardiac arrest brain injury, post-cardiac arrest myocardial dysfunction, systemic ischemia-reperfusion response, and the underlying precipitating pathology. Management focuses on targeted temperature management, hemodynamic optimization, ventilatory support, and treatment of the underlying cause. The severity and duration of PCAS correlate with the length of cardiac arrest and quality of resuscitation.
