A massive hemorrhage protocol (MHP) is a systematic approach to managing patients with severe bleeding, typically activated when blood loss exceeds 150ml per minute or one blood volume within 24 hours. The protocol coordinates rapid delivery of packed red blood cells, fresh frozen plasma, and platelets in predetermined ratios (often 1:1:1) to prevent coagulopathy. MHP activation involves multiple departments including blood bank, anesthesia, and surgical teams working in concert to address both the source of bleeding and the physiologic consequences of hemorrhage.
