Full Definition

A specialized computed tomography imaging protocol that captures images at multiple time points after contrast administration to identify parathyroid adenomas based on their characteristic enhancement pattern. The 'fourth dimension' refers to time-based imaging that shows the rapid uptake and washout of contrast in hyperfunctioning parathyroid tissue compared to normal thyroid tissue and lymph nodes. This technique has become increasingly important for preoperative localization of parathyroid adenomas, particularly in cases where other imaging modalities have been inconclusive or for reoperative parathyroid surgery.
