Parathyroid adenomatosis refers to the presence of multiple parathyroid adenomas (typically two or three glands) in a patient with primary hyperparathyroidism. This condition is distinct from both single adenoma disease and four-gland hyperplasia. It represents an intermediate form of parathyroid pathology that can be challenging to diagnose preoperatively and may require different surgical approaches. Patients with adenomatosis may have a higher risk of persistent or recurrent hyperparathyroidism if all abnormal glands are not identified and removed. The condition may be associated with genetic syndromes or may occur sporadically.
