The epileptogenic zone is the theoretical cortical area that is indispensable for seizure generation and whose complete removal or disconnection would result in seizure freedom. This zone cannot be directly measured but is inferred from multiple data sources including seizure semiology, neuroimaging, EEG, and invasive monitoring. It differs from related concepts such as the seizure onset zone (where seizures begin) or irritative zone (where interictal spikes occur). Accurate delineation of the epileptogenic zone is the primary goal of presurgical evaluation and determines surgical success.
