IPAA (ileal pouch-anal anastomosis) is a complex reconstructive procedure performed after total proctocolectomy to restore bowel continuity while maintaining fecal continence. The procedure involves creating a reservoir or 'pouch' from the terminal ileum, which is then connected directly to the anal canal. This allows patients to avoid a permanent ileostomy. The most common pouch configurations are J-pouch, S-pouch, or W-pouch designs. A temporary diverting ileostomy is often created during the initial surgery to protect the anastomosis during healing, with ileostomy closure performed several months later.
